“Big Data” involves delivering data-driven insights using robust high-scale computing systems to analyze large volumes of both structured and unstructured data. As the US healthcare system continues to undergo a major transformation initiated by healthcare reform, data will become critical for understanding the “who, what, where, why and how” of actually improving outcomes while reducing cost and variability. Studies show that 90% of healthcare data is not coded or structured but is in the form of clinical narrative (encounter notes, procedure notes, images, etc.). Furthermore, over 63% of key clinical information resides only in this unstructured data layer, essentially hidden from systems that rely upon coded data.
In this presentation, Apixio's CMO Dr. Darren Schulte will describe how Big Data analytics can improve problem list accuracy, risk prediction, and quality measurement, all of which is fundamental for optimal patient care and effective population management.
To achieve success, ACOs will need to realize savings by reducing inappropriate or duplicative services and applying evidence-based practices in a coordinated fashion across the attributed population. As a starting point, it is essential to assemble a consistent and complete view of a patient’s clinical history for efficient access by all care providers. In addition, data intelligence is needed to establish patient risk profiles, treatment adherence, and clinical and cost trajectories to inform care management. Despite the wider adoption of electronic records, patient information remains fragmented and inaccessible, largely hidden in clinical documents and other unstructured data.
Join our CMO Dr. Darren Schulte in this webinar to learn about Apixio's Clinical Knowledge Exchange, an intelligent cloud-based platform that can aggregate, index and enrich patient data (both structured and unstructured), apply sophisticated analytics, and route key insights directly to clinical workflow and reporting applications to achieve optimal quality and cost objectives for an ACO.
Significantly Improve Your Medicare Advantage Reimbursement : Using Intelligent Analysis of the Clinical Record
Medicare Advantage (MA) plans and providers spend much time, effort and resources on activities to properly risk adjust for beneficiaries given their burden of chronic disease. Many organizations audit MA beneficiary medical charts to avoid missing coding opportunities during each HCC reporting year. This may cost anywhere from $30 to $50 or more per chart. While there are many claims-based vendor solutions to uncover suspect diagnosis codes and to better target potential audit and documentation gaps, these tools miss many opportunities only found in the clinical record.
Apixio will describe innovative and intelligent analytics to automatically expose actionable opportunities to improve risk adjustment factor scoring by automatically extracting relevant diagnoses in clinical narrative using both text and scanned documents and comparing them against claims data to identify information gaps. The model leverages big data analytic techniques to determine where information gaps which can be addressed by coders to improve both chart audit productivity and also uncover additional opportunities for reimbursement.
To succeed, ACOs will need to rapidly deploy tools that will enable care teams to have efficient and instant access to patient information. To tackle the changing healthcare environment ACO’s will need to deploy lightweight, cloud based data infrastructure systems that liberates the data and makes it available to all care participants and other information systems on the care team. Apixio accomplishes this by leveraging the intelligent Clinical Knowledge Exchange to provide the narrative related to a recent hospitalization to a care coordinator as well as the ability to mine the narrative for discrete data opportunities that can then be fed to quality, risk and revenue tools. Creating a single aggregated view of a patient is expeditiously implemented across healthcare systems so that ACOs may be ready to roll at the start of the ACO period.
Big Data in Healthcare - A Big Data scientist’s perspective on a highly scalable platform for population Analytics
Big Data is the BIG buzzword in the health industry. What is Big Data and more importantly what infrastructure do you need to handle this data? The other questions to keep in mind will be how to then effectively use the data to answer the need for better patient healthcare management.
Join our Principal Scientist, Vishnu Vyas in this webinar to learn about Apixio's Clinical Knowledge Exchange, an intelligent cloud-based platform that allows for population level queries and analytics to be carried out effectively with great results using a semantic analysis pipeline built on top of Hadoop and Cassandra to aggregate, index and enrich patient data (both structured and unstructured), apply sophisticated analytics, and route key insights directly to clinical workflow and reporting applications to achieve optimal quality and cost objectives.
Chinese Community Health Care Association of San Francisco, CA is a non-profit IPA focusing on the care of the Chinese community. With a large Medicare Advantage population, accurate HCC Coding is imperative to the success of CCHCA in being able to properly care for their patients and reinvest within the local community.
Join HIT Industry Professional Jonathan Everett in this webinar, where he discusses how, through the use of Apixio’s solutions, CCHCA is finding new ways to search the narrative of their medical records and find opportunities that have been missed in regards to HCC Coding. Apixio’s HCC Optimizer enables CCHCA to audit their medical records more efficiently with state of the art algorithms and ensure that they are receiving appropriate amounts of funding for their patient population.
Jonathan Everett is a seasoned HIT Industry Professional with over 10 years of experience directing, managing, developing and implementing Electronic Health Records systems. He has a vast level of experience in Ambulatory Care, Hospital Systems, Custody Health and Emergency Medicine. Jonathan’s focus in his career has been working with low-income communities and nonprofit organizations trying to adopt and implement technical solutions. As of late, his most recent project at Chinese Community Health Care Association in San Francisco, CA, was highlighted by Dr. Farzad Mostashari, The Chair of the ONC as being innovative and subsequently used as an example in many publications and Dr. Mostashari’s presentations.
